Whether its strategies, ideas, practical or emotional support so you can continue to do what you want, or connecting you with others who have similar experiences, we can help you get the support you need.
If you are interested in becoming a Blind Low Vision NZ client, click on the link/button ‘Register for Services Now’ below, to complete our online registration form. Please note, we will need the following information from you in order to begin your registration:
- Your contact details.
- Next of kin or alternative contact.
- The name of your Ophthalmologist or eye health practitioner.
- A copy of your most recent eye report (ideally as an electronic file).
If you don’t have a copy of your eye report, we suggest you contact your eye health practitioner first to obtain this. You can ask your practitioner to make an online ophthalmic referral on your behalf. This will provide us with the information we need to begin your registration.
